JDF 35 – This Application form can be used to make a written request to the State Court Administrator regarding the cancellation of information from the National Instant Criminal Background Check System (NICS). Please review the Application form and the applicable statutes for additional information. This form is not filed with the court. JDF 36 – This Petition can be used to ask the court for relief from federal firearms prohibitions imposed against you. Please review the Petition and the applicable statutes for additional information. This form should be filed with the court that is handling the case that has given rise to the firearms prohibitions imposed against you.
JDF 37 – This Order should be filed with the court in conjunction with JDF 36 (this Order should not be used with JDF 35). Fill out the caption portion only, and the court will fill out the rest of the Order.
